Senior Care Management

Our senior care management services provide personal care that respects our client’s humanity and ongoing support that keeps him or her engaged in favorite activities and life.

We work closely with the care recipient, family and other related providers to implement the solutions we have developed to meet immediate and long term care needs. We understand that change is hard for everyone. We are not there the make huge changes that disrupt everyone’s lives. Our approach is gentle, taking baby steps to introduce change so that everyone has time to adjust, trust us and trust their process.

Our care management services include:

When developing a senior care plan, we not only consider basic day-to-day care needs and ideal living situations, but we also look for ways for you to stay engaged and active in life and for family members to stay positively involved. Our assessment includes:

  • Implementation of our solution-based action plan to support care recommendations and gently support safe transition.
  • Advocacy at doctor appointments, hospitalization and rehabilitation centers to assure client’s needs are communicated and met and client understands discharge and/or medication orders.
  • Mobility support including strength training, physical therapy, range of motion exercises.
  • Assistance with safe discharge from a hospital setting, managing appropriate care at home or in an assisted living/dementia facility.
  • Transportation and accompaniment to all medical appointments and/or scheduled outings.
  • Oversight and management of private caregiver(s), companion or personal assistant to maintain quality assurance.
  • Monitoring and management of daily activities, caregiver guidelines, nutrition and meal planning and documentation.
  • Oversight of activity calendar to keep individual engaged with purpose.
  • Therapeutic activities.
  • Ongoing preferred communication with family members or appropriate DPOA.
  • Collaboration with health care professional to avoid future problems and conserve assets.
  • Help with government services to lessen drain on financial resources.
