Our 6-step Senior Care Assessment

When developing a senior care plan, we not only consider basic day-to-day care needs and ideal living situations, but we also look for ways for you to stay engaged and active in life and for family members to stay positively involved. Our assessment includes:

  1. Phone conference or in-person meeting with family and/or loved one to discuss immediate challenges and short and long term goals.
  2. In-home assessment to evaluate and identify strengths and limitations with social, physical, mental, emotional function and the ability to remain independent and safe in one’s environment.
  3. Solution-focused senior care action plan developed and discussed with family.
  4. Recommendations for appropriate care services and resources to support a safe continuum of care and a healthy transition.
  5. Ongoing communication with family members or Durable Power of Attorney as action plan is implemented.
  6. Psychosocial education, advocacy and ongoing validation so family members and senior care recipients feel supported.
