Family Consultation

Accepting and managing life transitions of the older adult often requires the engagement and support of the entire family. The initial consultation brings the family together as a team to discuss challenges and to focus on finding solutions. We uncover opportunities that will result in an ongoing care plan to begin a life changing journey toward maintaining autonomy, providing a safe environment and enhancing overall health. Individual coaching is available for family members.

Grief, Loss and Life Transition

The Aging AdultThe natural process of aging brings a series of transitions which are accompanied by feelings of loss. Uncomplicated grief encompasses a broad range of feelings and behaviors. Some most common to the older adult are the inability to maintain independence, loss of dignity, loss of family and friends through death and the decline of social, mental and physical function. How do you cope with the powerful emotions, chaos, and confusion that follow such a loss? What can you do to ease the pain?

Grief work is a courageous and healing process. In bereavement, it is important that we grieve in our own way and recover optimally to begin living again. For some this can be a difficult journey to walk alone without guidance and support.

Kathy Faenzi encourages the bereaved individual to come to terms with the loss by sharing and recognizing unresolved issues. She guides the individual through this process which can lead to greater self knowledge, strength and ultimately a sense of renewal. Individual and group counseling are available.

Clinical Assessment

An extensive evaluation with the older adult to determine strengths and limitations with social, physical, mental and emotional functions. A home safety assessment is also available. Based on the findings, care options and services will be recommended to promote appropriate solutions.

Care Management

Care management involves monitoring activities of living and personal care of the older adult. Whether short term or long term care, we work in conjunction with the family and other resources to assure safety, preserve autonomy and dignity, provide healthy nutrition, and promote both mental and physical stimulation. We advocate and assist with in- home care, companion care, medical appointments, medication management and social outings. We offer housing recommendations and assistance with transition when relocating from home to an appropriate assisted living, skilled nursing or dementia community.

Educational Workshops and Seminars

Kathy is available to facilitate workshops, join as a panelist and lead support groups. She offers topics to educate and encourage alternative interventions for the older adult dealing with life transition. Contact Kathy to inquire about her availability for your next meeting.
